    The transition from hospital to home can be a challenge. This presentation identifies some of the common enteral formula delivery challenges for the pediatric patient transitioning to the home setting and recommendations for how to overcome those challenges.

    • Describe initial challenges that can be encountered in the transition from hospital to home
    • Discuss enteral formula delivery challenges in home pediatric enteral nutrition support
    • Determine if the feeding regimen is successful
